29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. a) Jobsite: Pressing area - Employees were exposed to projectile hazards while operating the unguarded Industrial Tire Exchange Hydraulic Tire Press to mount tires. On February 9, 2018, while mounting a 12.00-20 Solideal industrial tire, the press cage fragmented into 3 pieces while under pressure from the tire press and struck two employees resulting in fatal injuries to an employee. Noted on or about 2/9/18.
