29 CFR 1926.1053(b)(7): Ladder(s) that were unsecured or not provided with slip-resistant feet were used on slippery surfaces:  (a) Super 1 Foods, on or about February 1, 2018, an unsecured extension ladder used by employees to access the second floor was placed on concrete that had accumulated sawdust.  NOTE: Abatement documentation and certification are required for this item.

29 CFR 1926.1053(b)(8): Ladders placed in a location where they could be displaced by workplace activities or traffic, such as in passageways, doorways, or driveways, were not secured to prevent accidental displacement, nor was a barricade used to keep the activities or traffic away from the ladder.  (a) Super 1 Foods, on or about February 1, 2018, an extension ladder was not protected against displacement as employees used it to access a second floor.   Note: Abatement certification and documentation are required for this item.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  (a) Super 1 Foods, on or about February 1, 2018, employees installing refrigeration equipment on the second floor were not protected from falling approximately 12 feet to the ground.  NOTE: Abatement certification is required for this item.
