The employee was not wearing a body harness/belt with an attached lanyard. The employee was exposed to a fall of approx. 25 to 30 ft. NO ABATEMENT DOCUMENTATION REQUIRED

29 CFR 1904.39(a)(2): The employer failed to report the in-patient hospitalization of one or more employees as a result of a work-related incident, within (24) twenty-four hours. a) On 2/12/18 on a jobsite at 8660 Stahley Road, Clarence Center, NY: An employee was injured in a work-related incident involving a fall from the bucket of a bucket truck while engaged in tree cutting/trimming work. The employee was transported by ambulance to the hospital and admitted as an in-patient on 02/12/18. Employer did not notify OSHA within 24 hours of a work related incident that resulted in an in-patient hospitalization. NO ABATEMENT CERTIFICATION REQUIRED
