29 CFR 1910.22(a)(i): Housekeeping: All places of employment, passageways, storerooms, service rooms, and walking-working surfaces were not clean, orderly, and in a sanitary condition.  In the store: On or about February 28, 2018, passageways, storerooms, service rooms, and walking-working surfaces were not clean, orderly, and in a sanitary condition in the following instances:  a) In the store, carpet floor in the store was not kept clean there was fluid stains and mold growth on carpet and ripped carpet floor at store entrance.  b) In the warehouse, floor aisles were obstructed by clothing racks, drop ceiling tiles were broken and/or missing and bird feces smeared on warehouse wall.  This condition exposed employees to trip and health hazards.      Fallas Paredes was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.22(a)(i), which was contained in OSHA inspection number 1260957, citation number 1, item number 1 and was affirmed as a final order on 03/13/2018, with respect to a workplace located at 8500 Dryer St. # 45, El Paso, TX. 79904.

29 CFR 1910.37(a)(3): Exit routes were not free and unobstructed. Materials or equipment were placed, either permanently or temporarily, within the exit route. The exit access went through a room that could be locked, to reach an exit or exit discharge.   In the warehouse: On or about February 28, 2018, cardboard boxes and racks with merchandise were obstructing the warehouse emergency door. This condition exposed employees to fire hazards and entrapment.

A continuing and effective extermination program was not instituted where their presence was detected.  In the store and warehouse areas: On or about February 28, 2018, the store and warehouse ceiling, walls and doors was not constructed, equipped, and maintained, so far as reasonably practicable, as to prevent the entrance or harborage of rodents, insects, and other vermin. This condition exposed employees health hazards.

29 CFR 1910.303(g)(1)(i)(A):The depth of the working space in the direction of access to live parts were not less than indicated in Table S-1. A minimum of three (3) ft. of clear working space is required to be maintained at Electric equipment 600 V or Less.  In the warehouse: On or about February 28, 2018, the clear working space of 3-feet on electrical panel "A" (120-v/208-v) and panel "CR"  (120-v/208-v) in the warehouse was not maintained. This condition exposed employees to fire and electrical shock hazards.

29 CFR 1910.157(c)(1): Portable fire extinguishers were not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries.  In the store located near the lady's department: On or about February 20, 2018, the store fire extinguisher located by lady's clothing department was not mounted, located and identified so that they were readily accessible without subjecting the employees to injuries. This condition exposed employees to fire and struck-by hazards.
