29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee used appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation.  a). At the worksite - On or about February 15, 2018, an employee operated a Hitachi pneumatic nail gun without the use of eye protection, thus exposing the employee to a struck-by hazard.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b).  a). At the worksite - On or about February 15, 2018, an employee worked from the structure's top plate without the use of a fall protection system in place, thus exposing the employee to a fall hazard of approximately 9.3 feet.

29 CFR 1926.1053(b)(13): The top or top step of a stepladder was used as a step.  a). At the worksite - On or about February 15, 2018, employees used the top cap and top step of a Louisville and a Werner fiberglass stepladder, thus exposing employees to a fall hazard of approximately 7.4 feet.
