29 CFR 1926.102(a)(1): The employer did not ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:  a) At the jobsite, on or about February 15, 2018, several employees were operating pneumatic nail guns, hand tools, and electric saws without the use of eye protection, thus exposing employees to a struck-by hazard.

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:   a) At the job site, on or about February 15, 2018, six employees were working from the roof removing and installing roof sheathing and shingles onto the roof deck without any means of fall protection, exposing employees up to 12 feet fall hazards.

29 CFR 1926.503(a)(2): The employer shall assure that each employee has been trained, as necessary, by a competent person:  a)  At the job site, on or about and times prior to February 14, 2018, the employer did not ensure employees were trained by a competent person on the fall protection requirements when conducting roofing activities, thus exposing employees to a fall hazard up to approximately 12 feet.
