On or about February 16, 2018, two employees using saws to cut wood were not provided eye protection and were exposed to eye injuries such as flying debris.

On or about February 16, 2018, Building #6, two employees installing roof shingles on a 4:12 pitch roof were not protected from falling 17 feet to the ground by the use of a fall protection system.
