On or about February 16, 2018, employees working on the ground underneath other employees on the roof were not wearing hard hats being exposed to head injuries.

29 CFR 1926.102(a)(1): The employer shall ensure that each affected employee uses appropriate eye or face protection when exposed to eye or face hazards from flying particles, molten metal, liquid chemicals, acids or caustic liquids, chemical gases or vapors, or potentially injurious light radiation:    a. On or about February 16, 2018, two employees using saws to cut wood were not provided eye protection and were exposed to eye injuries such as flying debris.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501(b):    a. On or about February 16, 2018, Building #19, nine employees removing an old roof system on a 4:12 pitch roof were not protected from falling 17 feet to the ground by the use of a fall protection system.     b. On or about February 16, 2018, Building #6, two employees installing roof shingles on a 4:12 pitch roof were not protected from falling 17 feet to the ground by the use of a fall protection system.
