29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: On or about February 20, 2018, at the Contech collator machines the elevated walking platforms were not guarded on the inside to prevent a fall. There was no top railing on all three machines to prevent the employees from falling to the lower level. Employees exposed to a fall hazard greater than 4 feet to the cement floor below.
