29 CFR 1926.451(g)(1)(i): Each employee on a boatswains' chair, catenary scaffold, float scaffold, needle beam scaffold, or ladder jack scaffold was not protected by a personal fall arrest system: a) Jobsite, 101 Mikula Road, Belle Vernon , PA 15012: On or about November 26, 2017, each employee working on a ladder jack scaffold was not protected from falling by a personal fall arrest system. An employee working on a ladder jack scaffold located on the west side of a two story structure was exposed to a fall hazard of approximately 16 feet to the ground below.
