29 CFR 1926.102(a)(1): Eye and face protective equipment were not used when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:     On or about February 20, 2018, and times prior thereto, at the jobsite located at 3321 S Bowman Road, Little Rock, AR, an employee was observed using a Hitachi pneumatic nail gun without the use of eye protection.

29 CFR 1926.1053(b)(1): Portable ladders were used for access to an upper landing surface and the ladder side rails did not extend at least 3 feet (.9 m) above the upper landing surface to which the ladder was used to gain access:     On or about February 20, 2018, and times prior thereto, at the jobsite located at 3321 S Bowman Road, Little Rock, AR, an employee ascended and descended from the rooftop on an aluminum extension ladder which did not extend at least three feet above the rooftop landing, exposing the employee to fall hazards greater than 6 ft. to the ground below.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):     The Antonio Dominguez was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.501(b)(13), which was contained in OSHA inspection number 1242449, citation number 1, item number 2 and was affirmed as a final order on July 21, 2017, with respect to a workplace located at 9308 Johnson Drive, Sherwood, AR 72120.    On or about February 20, 2018, and times prior thereto, at the jobsite located at 3311 S Bowman Road, Little Rock, AR each employee observed working on a residential roof, did not have fall protection.
