29 CFR 1904.8(a): For work-related needlestick injuries and cuts from sharp objects that are contaminated with another person's blood or other potentially infectious material, the employer entered the employee's name on the OSHA 300 Log:  a) BWXT, 1570 Mt. Athos Rd, Lynchburg, VA 24504: On or about February 21, 2018, and times prior thereto, the employer made an entry on the OSHA 300 Log of a needlestick injury incurred by an employee on February 9, 2018. The employer did not enter "Privacy case" or equivalent in lieu of the employee's name to protect the employee's privacy.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1904.32(b)(2)(ii): The annual summary did not include one or more of the following elements: the calendar year covered, the company's name, establishment name, establishment address, annual average number of employees covered by the OSHA 300 Log, and the total hours worked by all employees covered by the OSHA 300 Log:  a) BWXT, 1570 Mt. Athos Rd, Lynchburg, VA 24504: The employer combined the 2015, 2016, and 2017 summary data on a non-equivalent form. The 2017 OSHA 300A Summary or equivalent form did not include the establishment address, number of employees and total hours worked or record the type(s) of injuries.   Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.

29 CFR 1910.132(d)(2): The employer did not verify that the required workplace hazard assessment has been performed through a written certification which included the requirements as outlined in 29 CFR 1910.132(d)(2):  a) BWXT, 1570 Mt. Athos Rd, Lynchburg, VA 24504: On or about February 21, 2018, and times prior thereto, the employer did not have a written certification of a site-specific hazard assessment for the necessary and appropriate PPE for janitorial employees.  Abatement certification required within 10 days after abatement date. The certification shall include a statement that abatement is complete, date and method of abatement, and states employees and their representatives were informed of this abatement.
