29 CFR 1926.100(a): Employees working in areas where there was a possible danger of head injury from impact, or falling or flying objects, or from electrical shock and burns, were not protected by protective helmets:     On or about February 21, 2018, employees working in an area where overhead hazards existed were not protected by hard hats, exposing the employees to a struck-by hazard.     Abatement certification is required of this item in accordance with the requirements of 29 CFR 1903.19(c).

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.    On or about February 21, 2018, employees were not protected from a fall while walking/ working from the roof of a commercial building, exposing employees to a fall of over 25 feet.     Abatement documentation is required of this item in accordance with the requirements of 29 CFR 1903.19(d).
