29 CFR 1910.147(c)(4)(ii): The procedures shall clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, and the means to enforce compliance. a. On or about February 19, 2018, and at times thereto, the employer did not develop and document a machine specific energy control procedure for authorized employees to utilize when performing service and maintenance activities on the MASA pump system for the 1,000 Corn Line. Such activities included flushing out the MASA pump piping to the sheeter at the end of production runs and prior to shutdowns.

29 CFR 1910.147(d)(3): Application of control. The established procedures for the application of energy control (the lockout or tagout procedures) shall cover the following elements and actions and shall be done in the following sequence as required by 29 CFR 1910.147(d)(1) through (d)(6): a. On or about February 19, 2018, and at times thereto, employees were exposed to a caught in hazard while flushing out the MASA pump and piping feeding the 1,000 pound sheeter. The employer did not ensure that energy control application steps were implemented as the electrical energy was not isolated from its source [per the 1910.147(d)(3) requirements]. As a result, the remaining applicable energy control elements, lockout/tagout devices were not applied to the disconnect equipment [(d)(4)], dissipation of the residual energy of the pump motor [(d)(5)(i)], and verification of isolation [(d)(6) were not implemented to protect employees from the pump servicing hazard.
