29 CFR 1910.147(c)(6)(ii): The employer did not certify that periodic inspections of the energy control procedures had been performed:     Employees engaged in servicing, maintenance, setup, clearing jam activities in the warehouse areas at the following equipment but not limited to the B1 palletizer machine, and the B2 palletizer machine which had multiple energy sources including pneumatic, electric, and gravity were exposed to electrical, struck-by and caught-between hazards in that periodic inspections of the energy control procedures for equipment were not performed nor documented.    29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.178(a)(6): Nameplates or markings for powered industrial trucks were not in place or were not maintained in a legible condition:    Employees engaged in forklift operation and material handling activities in the lower warehouse areas were exposed to struck-by hazards in that identifying markings were absent from the control levers to indicate the actions the levers performed for the following powered industrial trucks:    i) the Toyota Forklift #22, the slip sheet attachment indicator lever was not labeled.  ii) the Toyota Forklift #26, the slip sheet attachment indicator lever was not labeled.  iii) the Toyota Forklift #5, the slip sheet attachment indicator lever was not labeled.    29 CFR 1903.19(c)(1) requires certification that the abatement of the above violation is complete.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:    Employees engaged in machining activities at the B2 palletizer in the lower warehouse area were exposed to struck-by and amputation hazards in that machine guarding was not provided for the B2 palletizer, thus allowed the employee to reach his hand inside the open maintenance hole to remove the bag in order to clear the jam.    29 CFR 1903.19(c)(2): Abatement of the above violation was verified at time of inspection, no certification is required.

29 CFR 1904.39(a)(3): The employer did not report the fatality, in-patient hospitalization, amputation, or loss of an eye either by telephone, to include the OSHA toll-free number, in-person, or by electronic submission:  On or about December 28, 2017 - An employee was injured in the workplace requiring a hospital stay and was not reported to OSHA until February 14, 2018.  29 CFR 1903.19(c)(2): Abatement of the above violation was verified at time of inspection, no certification is required.
