29 CFR 1910.134(a)(2): A respirator was not provided by the employer to each employee when such equipment was necessary to protect the health of the employee: a) In the old large tank shop and new large tank shop, on or about 3/16/18: The employer did ensure that employees used respirators when employees conducted manual grinding on stainless steel tanks. OSHA air sampling results showed an employee conducting grinding on tanks was exposed to 181% of the Permissible Exposure Limit to the mixture of Vanadium, Cobalt, Nickel, Copper, Iron Oxide, Manganese, and Chromium. Abatement certification must be submitted for this item.

29 CFR 1910.1000(d)(2)(i): An employee was exposed to Vanadium, Cobalt, Nickel, Copper, Iron Oxide, Manganese, and Chromium listed in the Z tables in excess of the cumulative exposure limit: a) In the old and new large tank shops, on or about 3/16/18: An employee who conducted manual grinding on stainless steel tanks was exposed to 181% of the Permissible Exposure Limit to the mixture of Vanadium, Cobalt, Nickel, Copper, Iron Oxide, Manganese, and Chromium. The sampling period was for 390 minutes. A zero increment was included for the 90 minutes not sampled. Abatement certification must be submitted for this item.
