One or more methods of machine guarding shall be provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks. Examples of guarding methods are-barrier guards, two-hand tripping devices,electronic safety devices, etc. a. On or about February 22, 2018, the employer located at 5700 Lands' End Way Stevens Point, WI , did not ensure that employees were protected from an ingoing nip point at the feed end of the Amscomatic K-840 Folder & Bagger Unit.

29 CFR 1904.41(a)(1): Annual electronic submission of part 1904 records by establishments with 250 or more employees. You must submit the information once a year, no later than the date listed in paragraph (c) of this section of the year after the calendar year covered by the forms. a. On or about February 22, 2018, the employer who had over 250 employees during 2017 and was in an industry listed in appendix A to subpart E of this part, did not electronically submit their OSHA Form 300A to OSHA prior to the due date of December 15, 2017.
