29 CFR 1910.95(k)(1): The employer did not train each employee who is exposed to noise at or above an 8-hour time-weighted average of 85 decibels in accordance with the requirements of 29 CFR 1910.95(k):    (a) Saw and Repair Rooms - The employer did not provide noise training to each employee repairing the pallets where their noise exposure exceeded 85 decibels, on or about February 21, 2018.    No abatement certification or documentation required.

29 CFR 1910.95(m)(2)(ii): The employee audiometric test records did not include at least the employee's name and job classification, the date of the audiogram, the audiogram, the examiner's name, the date of the last acoustic or exhaustive calibration of the audiometer, and the employee's noise exposure assessment result:    (a) Saw Room and Repair Room - The employer's audiometric test records for calendar year 2017 did not contain the employee's job classification and the noise exposure assessment result, on or about February 21, 2018.  No abatement certification or documentation required.
