29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks:     In the following locations, adequate machine safeguarding had not been provided:      A.  Building 1, 967 Parker Street, Sheetmetal Fabrication:  On, or about, February 21, 2018, the emergency stop cable provided on the Cemco sheetmetal shrinker/expander was installed too close to the machine frame.  The cable could not move freely if activated.      B.  Building 4, 255 Sheldon Street, Blade Track Department:  On, or about, February 21, 2018, the light curtain provided on machine #752, an Amada RG-355 hydraulic press brake was positioned too high, allowing access to the point of operation by reaching under the sensing field.    C.  Building 2, 203 Sheldon Street:  On, or about, February 22, 2018, an access door provided on the cutting head enclosure on the Mazak Super Turbo X48 laser cutter allowed access to the area where the cutting head traversed, created a pinch point.      D.  Building 3, 186 Adams Street, Weldments:  On, or about, February 22, 2018, the auger-type chip collector provided on Machine 382, a Haas CNC milling center, could run with the access door open.    E.  Building 3, 186 Adams Street, NPC Department:  On, or about, February 22, 2018, the auger-type chip collector provided on the Haas VF3 CNC milling center could run with the access door open.      F.  Building 3, 186 Adams Street, GE Cell:  On, or about, February 22, 2018, the auger-type chip collector provided on Machine #1261, a Haas CNC milling center, could run with the access door open.

29 CFR 1910.217(c)(3)(iv)(d): Pullout device(s) on mechanical power press(es) were not visually inspected and checked for proper adjustment at the start of each operator shift, following a new die setup, and when operators were changed:  Building 1, 967 Parker Street, Liner Cell:  On, or about, February 21, 2018, the company did not provide a second person to assist the Pressman to verify the proper set-up of Woodbury pull-back devices on the five full revolution mechanical power presses before each shift.  The pull-back devices could not be properly verified by the operator alone.

29 CFR 1910.217(b)(6)(ii): Two-hand trip system(s) on mechanical power press(es) using full revolution clutches did not incorporate an anti-repeat feature:   Building 1, 967 Parker Street, Liner Cell:  On, or about, February 21, 2017, Press #73, a Niagara full revolution mechanical power press, was provided with two hand trips to cycle the press.  The press cycled twice when the trip buttons were held down.
