29 CFR 1910.253(b)(4)(iii): Oxygen cylinders in storage were not separated from fuel-gas cylinders or combustible materials (especially oil or grease), a minimum distance of 20 feet (6.1 m) or by a noncombustible barrier at least 5 feet (1.5 m) high having a fire-resistance rating of at least one hour: Employees engaged in cylinder handling and storage operations were exposed to fire hazards in that the oxygen cylinders were allowed to be stored on the opposite side of a steel barrier that was not properly constructed to provide a 1 hour fire rating.

Employees engaged in forklift charging operation in the south plant were exposed to shock hazards in that the electrical panel that served that forklift charger had multiple openings exposing live bus bars inside the panel.
