29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a)  At Lot 16 Saratoga Loop, Churchill Subdivision, Spanish Fort, Alabama:  On or about February 23, 2018 and at times prior; the employer failed to ensure his employee decking a high slope roof was using fall protection properly exposing the employee to a fall hazard of 9 feet 5 inches to the ground below.

29 CFR 1926.1053(b)(13): The top or step of a stepladder was used as a step:   a)  At Lot 16 Saratoga Loop, Churchill Subdivision, Spanish Fort, Alabama:  On or about February 23, 2018 and at times prior; the employer allowed his employee to use the top step of a six foot a-frame ladder to gain access to the roof top exposing the employee to a fall hazard of 6 ifeet to the ground below.
