29 CFR 1910.67(c)(2)(v): A personal fall arrest or travel restraint system that meets the requirements in subpart I of this part was not worn and attached to the boom or basket when working from an aerial lift: (a) On or about February 22, 2018, Lumber Bee Tree & Lawn Care, LLC did not ensure that employees working from an Altec GMC Boom Truck serial no. 2728683631104, approximately 10 feet above the ground were protected from fall hazards by using a personal fall arrest or restraint system while conducting tree trimming operations. Abatement Note: Abatement certification and documentation are required for this item (see enclosed "Certification of Corrective Action Worksheet"). Abatement Note: The employer should ensure that when a fall arrest system/restraint is utilized in a aerial boom platform that the lanyard and anchor are arranged so that the employee is not exposed to falling any distance outside the working platform or to a lower level.
