29 CFR 1926.102(a)(1): Employees were not provided with eye and face protection equipment when machines or operations presented potential eye or face injury from physical, chemical, or radiation agents:  (a) West Wall of Lot Number 19 Addison Subdivision: On or about February 22, 2018, and at times prior, the employer exposed an employee to struck-by hazards while using a pneumatic nail gun, in that protective eyewear was not provided to the employee operating the tool at 120 PSI while engaged in siding installation activities.

29 CFR 1926.501(b)(13): Each employee(s) engaged in residential construction activities 6 feet (1.8 m) or more above lower levels were not protected by guardrail systems, safety net system, or personal fall arrest system, nor were employee(s) provided with an alternative fall protection measure under another provision of paragraph 1926.501 (b):   a) West Wall of Lot Number 19 Addison Subdivision: On or about February 22, 2018, the employer exposed employees to fall hazards of approximately 13 feet while installing siding material on the side wall of a single family construction site without proper fall protection.
