29 CFR 1926.351(a)(2): Current carrying parts of manual electrode holder(s) passing through the gripping portion or the outer surfaces of the jaws were not fully insulated against the maximum voltage encountered to ground: a) Fort Pickett: On or about 02/21/2018, an employee used an electrode holder to weld tie-back clips between the first and second floors of the building. The electrode holder was missing a tip insulator used to insulate the employee from a possible electrical shock hazard. No abatement certification or documentation required.
