29 CFR 1926.405(a)(2)(ii)(F): Temporary lights were suspended by their electric cords when such lights and cords were not designed for suspension: a. On or about February 15, 2018 Employees working inside the elevator shaft had suspended a home made temporary lighting system from a sheet rock screw affixed to the side wall of the shaft on the third floor and draped the wires down the shaft to the first floor. At each light socket spliced to the wire a sheetrock screw affixed the wires to the wall.

29 CFR 1926.405(a)(2)(ii)(J): Flexible cords used with temporary and portable lights were not designed for hard or extra-hard usage: a. On or about February 15, 2018 Employees had utilized a home made temporary lighting system constructed with 16 gauge, 2 conductor non grounded SPT2 lamp wire, with medium base 2 wire temporary light sockets. The electric lamp wire was not deigned for hard or extra hard usage.

On or about February 15, 2018 Employees working outside the elevator shaft on the 3rd and 2nd floors, were not protected by a guardrail system complete with midrails, exposing them to falls in excess of 30 feet to the pit floor below.

On or about February 15, 2018 employees working outside the elevator shaft were not protected from falling more than 30 feet to the elevator pit because the guardrail system positioned at the shaft openings were not affixed to the walls or to the shaft opening and could not withstand 200 pounds applied laterally to the guardrail system.
