29 CFR 1910.147( d):  The established procedure for the application of energy control (the lockout or tagout procedure) was not done in sequence as required by 29 CFR 1910.147( d)(1) through (d)(6 ):     On or about February 14, 2018, employee was exposed to machine hazards associated with moving parts and in-going nip points during belt changing operations on the belt drive for the conveyor located at the river bank.  The employer failed to implement energy control application steps as LOTO devices were not applied to the electrical disconnect [per the 1910 .147(d)(4) requirements].

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used only in continuous lengths without splice or tap:    A flexible 480 volt cable was used to power the motor on the conveyor.  The cable was spliced with wing nuts and the cable's outer insulation was missing.

The employee was hospitalized for 3 days and the employer failed to report the hospitalization.
