29 CFR 1926.451(e)(1): When scaffold platforms were more than 2 feet (0.6 m) above or below a point of access, portable ladders, hook-on ladders, attachable ladders, stair towers (scaffold stairways/towers), stairway-type ladders (such as ladder stands), ramps, walkways, integral prefabricated scaffold access, or direct access from other scaffold, structure, personnel hoist, or similar surface was not used.  Crossbraces were used as a means of access:  On or about 2/22/2018 at 1323 W. Augusta Parkway Augusta, GA workers were exposed to fall hazards up to 22-feet above the concrete floor where observed accessing and egressing the scaffold during CMU block installation activities by way of the cross bracing and frame where no ladders were installed.

29 CFR 1926.451(h)(1): Each employee on a scaffold was not provided with additional protection from falling hand tools, debris, and other small objects through the installation of toeboards, screens, guardrail systems, or through the erection of debris nets, catch platforms, or canopy structures that contain or deflect the falling objects.  For falling objects too large, heavy or massive to be contained or deflected, the employer did not place and secure such potential falling objects away from the edge of the surface from which they could fall:  On or about 2/22/2018 at 1323 W. Augusta Parkway Augusta, GA workers performing CMU block installation activities that were on the ground below the scaffold were exposed to struck-by hazards where there were no toe boards installed. Potentially dropped items included but were not limited to block, hand tools, and materials.

29 CFR 1926.452(c)(4): End frames of tubular welded frame scaffolding were not locked together vertically by use of pins or equivalent means in areas where uplift could occur:  On or about 2/22/2018 at 1323 W. Augusta Parkway Augusta, GA workers on the scaffold were observed performing CMU block installation activities that were exposed to fall hazards above 22-feet to the concrete flooring below where the connecting pins were not installed in the scaffold being loaded by the JCB 507 forklift.
