OSH ACT of 1970 Section (5)(a)(1): The employer did not furnish employment and a place of employment which were free from recognized hazards that were causing or likely to cause death or serious physical harm to employees (a) At the job site - On or about February 2, 2018 an employee was exposed to a fall hazard while descending from the roof into the Barecat Series Scissor lift. Among other methods, feasible and acceptable abatement methods to correct this hazard include the following: comply with the American National Standard for Self-Propelled Elevating Work Platforms (ANSI-SIA A92.6-2006)

29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels were not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems. (a) At the worksite - On or about February 26, 2018, an employee was working at the roof close to the edge about 19 feet from the ground without any means of fall protection.
