29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems.  Worksite: The employer did not ensure that the employee who was working from the roof of a commercial building was protected from the fall hazards above 6 feet by the use of guardrails, safety nets or a personal fall arrest system.

Worksite: The employer did not ensure that the employee who was exposed to fall hazards above 6 feet while working from the roof of a commercial building was trained in fall hazards and the procedures to be followed to minimize such hazards.
