29 CFR 1926.651(c)(2): A stairway, ladder, ramp or other safe means of egress was not located in trench excavations that were 4 feet (1.22 m) or more in depth so as to require no more than 25 feet (7.62 m) of lateral travel for employees: a) Job site, near corner of South 13th Street and East Carson Street, Pittsburgh, PA: On or about February 26, 2018, during catch basin construction related activities, the employer did not ensure that a safe means of egress existed in an excavation determined to be approximately 6 feet deep. Independent Enterprises Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1926.651(c)(2), which was contained in OSHA inspection number 958963, citation 2, item 1, and was affirmed as a final order on May 5, 2014, with respect to a workplace located at a job site near 520 Aspen Road, Pittsburgh, PA.

Independent Enterprises Inc. was previously cited for a violation of this occupational safety and health standard or its equivalent standard, 29 CFR 1926.652(a)(1), which was contained in OSHA inspection number 1110250, citation number 2, item number 1, and was affirmed as a final order on April 1, 2016 with respect to a workplace located at 1122 Sheffield Street, Pittsburgh, PA.
