29 CFR 1910.22(d)(1): In every building or other structure, or part thereof, used for mercantile, business, industrial, or storage purposes, the loads approved by the building official were not marked on plates of approved design which were not supplied and securely affixed by the owner of the building, or his duly authorized agent, in a conspicuous place in each space to which they relate.    In the main mill room above the break room area, the employer stored materials above the office without a load rating sign.

29 CFR 1910.305(g)(2)(ii): Flexible cords were not used in continuous lengths without splice or tap.   On the southeast side of the main mill shop, the employer spliced the air compressor power cord to an electrical wire without a junction box exposing employees to electrical shock hazards.

In the main mill shop, the employer did not tag the compressor out of service so not to inadvertently re-energize the equipment and the belt was not guarded exposing employees to caught-by hazards.

29 CFR 1910.305(b)(3)(ii): Boxes shall be closed by suitable covers securely fastened in place, boxes were not closed as required.   On the east side of the production area, electrical circuit panel box that energizes the air compressor and wood cutting mizers was left open where employee contact to inadvertent re-energization of machines and electrical shock hazards.

29 CFR 1910.305(g)(1)(iv)(A): Flexible cords and/or cables were used as a substitute for the fixed wiring of a structure.     On the southeast side of the main mill shop, electrical cable not ran into conduit and an orange extension cord was used as permanent wiring to energize equipment exposing employees to electrical shock hazards.
