29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) before use: a. On or about March 2, 2018, in the Building 132 Pilot Plant, the employer had not corrected corrosion, deteriorated bolts, and missing bolts on the flange of the Phosphorus Tri-Chloride port to the EK-01 reactor, a process vessel that received a Process Safety Management listed chemical.
