On or about February 27, 2018, on the southeast side of a 4/12 pitch roof of a residential building, two employees working on a roof were not protected from a 21 feet 9 inch fall hazard by the use of a personal fall protection system. b. On or about February 27, 2018, on the southwest side of a 7/12 pitch roof of a residential building, three employees working on a roof were not protected from a 9 feet 5 inch fall hazard by the use of a personal fall protection system.

On or about February 27, 2018, on the southeast side of a 4/12 pitch roof of a residential building, two employees using vertical lifelines were working on a roof and were both attached to the same anchor point, the employees were not protected from a 21 feet 9 inch fall hazard.
