29 CFR 1910.147(c)(4)(i): Procedures were not developed, documented and utilized for the control of potentially hazardous energy when employees were engaged in activities covered by this section.     Pail Labeler:  On or about February 14, 2018, an energy control procedure was not utilized while an employee unjammed a pail at the hold down belt area of the line exposing the employee to amputations and in going nip point hazards.     In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.147(c)(4)(ii): The energy control procedures did not clearly and specifically outline the scope, purpose, authorization, rules, and techniques to be utilized for the control of hazardous energy, including, but not limited to Items of this section.    Pail Labeler: On or about February 14, 2018, the established general energy control procedure was not machine specific for the Pail line Heissler Labeler and did not clearly and specifically outline the techniques to lock out the machine such as including steps of where to place a lock.   In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.212(a)(1): One or more methods of machine guarding was not provided to protect the operator and other employees in the machine area from hazards such as those created by point of operation, ingoing nip points, rotating parts, flying chips and sparks.     a) On or about February 14, 2018, employees operating the Heisler Pail Labeler, were exposed to ingoing nip points at the hold down belt area because the belt and pulleys were not guarded effectively, allowing employees to access where the belt meets the pulleys during operation.      In accordance with 29CFR 1903.19(d), abatement certification is required for this violation (using CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation that abatement is complete must be included with your certification. This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement or other written records.

29 CFR 1910.147(c)(6)(i): The employer did not conduct a periodic inspection of the energy control procedure at least annually to ensure that the procedure and the requirements of this standard are being followed.   On or about February 14, 2018, a periodic inspection of the energy control procedure was not conducted in the past 12 months.    Certification, with date and method of abatement required.  Certification of abatement is required by 29 CFR Part 1903 and is to be submitted within (10) calendar days after the abatement date.
