Test voltages and the maximum intervals between tests shall be in accordance with Table I-4 and Table I-5:  On or about 24 February 2018, the employer did not ensure that the rubber insulating gloves issued for employee use had been electrically tested (or replaced) at 6 month intervals. The gloves had an inspection date of July 2012.  Employees were exposed to electric shock hazards when performing electrical work.  Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).

Employees who gained access to equipment during servicing and maintenance were exposed to caught-in hazards.   Abatement certification is required for this item in accordance with 29 CFR 1903.19(c).
