29 CFR 1926.501(b)(11): Each employee on a steep roof with unprotected sides and edges 6 feet (1.8 m) or more above lower levels was not protected from falling by guardrail systems with toeboards, safety net systems, or personal fall arrest systems:  a) For employees working at 605 SW 49th Lane in Cape Coral on or about February 27, 2018.  Employees were installing new roofing shingles on the 6:12 pitch roof without the use of any type of fall protection system in place. The employees were exposed to a fall height greater than 8 feet.   b) For employees working at 609 SW 49th Lane in Cape Coral on or about February 27, 2018.  Employees were installing new roofing shingles on the 6:12 pitch roof without the use of any type of fall protection system in place. The employees were exposed to a fall height greater than 8 feet.

29 CFR 1926.405(g)(2)(iv): Flexible cords were not connected to devices and fittings so that strain relief is provided to prevent pull from being directly transmitted to joints or terminal screws:  b) For employees working at 609 SW 49th Lane in Cape Coral on or about February 27, 2018.  Employees were using a portable power tools on the roof that were powered by an extension cord that was not provided with strain relief at the plug end. The outer insulation was pulled back approximately 1 1/2 inch exposing the inner insulated conductors. The employees were exposed to a electrical shock.
