West Hampton Dunes, NY, 11978, first level construction: Employees were using an extension ladder to access the first level construction at approximately 11ft. from the ground below. The ladder side rails were extended approximately 30in. above the landing surface, on or about 2/27/2018. b) Worksite - 67 Dune Rd. West Hampton Dunes, NY, 11978, second level construction: Employees were using an extension ladder to access the second level construction at approximately 11ft. from the first level. the ladder side rails were extended approximately 3in. above the landing surface, on or about 2/27/2018.
