29 CFR 1910.147(c)(6)(i)(C): Where lockout was used for energy control, the periodic inspection did not include a review, between the inspector and each authorized employee, of that employee's responsibilities under the energy control procedure being inspected:    On or about February 26, 2018, the periodic inspection performed by the employer did not review each authorized employee's responsibilities under the energy control procedure used by employees in the compounding department to lockout the Dicer 1848 for cleaning during each shift.

29 CFR 1910.147(c)(7)(iii)(A): Retraining was not provided for authorized and affected employees when there was a change in their job assignments, a change in machines, equipment or processes that presented a new hazard, or when there was a change in the energy control procedures:  a. On or about February 26, 2018 the employer did not retrain the authorized employees working in the compounding department after updating the lock out location specified in the energy control procedure for the Dicer 1848 on December 11, 2017.   b. On or about February 26, 2018, the employer did not retrain the authorized employees working in the compounding department after adding two additional energy isolation locations in the energy control procedure for the mill on January 3, 2018.

Training to familiarize employees with the general principles of fire extinguisher use and the hazards involved with incipient stage fire fighting was not provided to all employees upon initial employment and at least annually thereafter. Hands-on training familiarizing employees with general principles of fire extinguishers was last held on October 11, 2016. Training on the company's fire extinguisher policy was last held on January 27, 2017.
