29 CFR 1910.137(c): In-service care and use of electrical protective equipment.    29 CFR 1910.137(c)(2): Specific requirements. The following specific requirements apply to rubber insulating blankets, rubber insulating covers, rubber insulating line hose, rubber insulating gloves, and rubber insulating sleeves:    29 CFR 1910.137(c)(2)(viii): Electrical protective equipment shall be subjected to periodic electrical tests. Test voltages and the maximum intervals between tests shall be in accordance with Table I-4 and Table I-5.    On or about February 27, 2018, the employer failed to assure that electrical protective gloves were subjected to electrical tests at the six month interval required by Table I-5 of this part. Electrical protective gloves had never been tested and were in use for a period exceeding six months. The electrical protective gloves were used by plant engineers during testing of live electrical parts and when working in close proximity to live electrical parts during required maintenance activities of diagnostic testing, repair, equipment isolation and preventive maintenance tasks.

29 CFR 1910.333(a)(2): "Energized parts." If the exposed live parts are not deenergized (i.e., for reasons of increased or additional hazards or infeasibility), other safety-related work practices shall be used to protect employees who may be exposed to the electrical hazards involved. Such work practices shall protect employees against contact with energized circuit parts directly with any part of their body or indirectly through some other conductive object. The work practices that are used shall be suitable for the conditions under which the work is to be performed and for the voltage level of the exposed electric conductors or circuit parts. Specific work practice requirements are detailed in paragraph (c) of this section.    On or about February 27, 2018, the employer had not implemented safety-related work practices for employees who may be exposed to electrical hazards covered by this part. Plant engineers had potential exposure to energized parts during required duties related to performing of preventive maintenance, equipment repairs and other diagnostic activities related to assigned work. These required duties with potential exposure to energized electrical circuits included, but was not limited to, pulling fuses related to electrical disconnect switch boxes during equipment repair and checking amperage on contactors during scheduled preventive maintenance.  The employer had not evaluated and documented the levels and type of electrical protective equipment and personal protective equipment, the types of electrical tools, testing equipment and developed task specific work practices to protect plant engineers during activities involving potential exposure to, and contact with, energized parts of electrical circuits.

29 CFR 1910.333(b): "Working on or near exposed deenergized parts."    29 CFR 1910.333(b)(2): "Lockout and Tagging." While any employee is exposed to contact with parts of fixed electric equipment or circuits which have been deenergized, the circuits energizing the parts shall be locked out or tagged or both in accordance with the requirements of this paragraph. The requirements shall be followed in the order in which they are presented (i.e., paragraph (b)(2)(i) first, then paragraph (b)(2)(ii), etc.).     Note 1: As used in this section, fixed equipment refers to equipment fastened in place or connected by permanent wiring methods. On or about February 27, 2018, the employer's written procedures for testing deenergized electrical parts, such as fuses related to electrical disconnect power supply circuits, did not include the specific testing procedure, the specific   testing equipment, electrical protective equipment and electrical personal protective equipment. Employees performing covered activities by this part, including but not limited to pulling fuses as part of performing repairs to a Cissell Tumbler, were exposed to potential electric shock hazards related to contact with electrical parts and deenergized electrical parts.

On or about February 27, 2018, the employer's energy control procedures did not provide specific procedural steps for isolating and securing hazardous energy sources for the Ellis "Washwheel" 900  during the performance of required duties covered by this part.  The written energy control procedure for this equipment, titled Ellis Washwheel 450 and 900, did not include the steps of multiple valve use for steam/water line energy control, bleeding of water/steam lines,  the use of the operator's controls in removal of energy from the lines and the specific sequence of use for the application of devices, bleeding of lines and use of the operator's controls for hazardous energy control.    During maintenance and repair activities performed by plant engineers, employees have potential exposure to thermal hazards, mechanical hazards and chemical hazards associated with required duties performed on a routine basis.
