29 CFR 1926.303(b)(1): Grinding machines were not equipped with safety guards in conformance with the requirements of ANSI B7.1-1970, Safety Code for the Use, Care, and Protection of abrasive wheels, and paragraph (d) of this section.    On or about February 28, 2018, on the north side of the building located at 1280 SW 26 Avenue, Davie, FL 33314, an employee was exposed to amputation hazards while using an unguarded grinder.

Or, on roofs 50-feet (15.25 m) or less in width, each employee was not protected by use of a monitoring system:    On or about February 28, 2016, on the north side of the building located at 1280 SW 26 Avenue, Davie, FL 33314, an employee was working on a roof without using a means of fall protection while exposed to an approximated 10-foot fall hazard.

29 CFR 1926.503(a)(1): The employer did not provide a training program for each employee potentially exposed to fall hazards to enable each employee to recognize the hazards of falling and the procedures to be followed in order to minimize these hazards:  On or about February 28, 2016, on the north side of the building located at 1280 SW 26 Avenue, Davie, FL 33314, an employee who had not been trained to recognize and avoid fall hazards was working from a roof without using a means of fall protection while exposed to an approximated 10-foot fall.
