On or about February 26, 2018, located at the Driven Sunbury in Sunbury, Ohio, employees were performing roofing work on a steep-pitch roof without any means of fall protection, thereby exposing the employees to a fall hazard of 9 to 24 feet. The employer never initiated or maintained safety and health programs to protect employees from the hazards associated with performing roofing work on a steep-slope roof.

On or about February 26, 2018, located at the Driven Sunbury in Sunbury, Ohio, employees were performing roofing work on a steep-pitch roof without any means of fall protection, thereby exposing the employees to a fall hazard of 9 to 24 feet. The employer never instructed employees in the recognition and avoidance of unsafe conditions with the regulations applicable to working on a steep-pitched roof in order to protect the employees from associated safety and health hazards.
