29 CFR 1910.212(a)(3)(ii): The point of operation of machines whose operation exposes an employee to injury, was not guarded as to prevent the operator from having any part of his body in the danger zone during the operating cycle: On or about; 2/26/18 Location: Fresh Produce Production Area, Scallion cutter machine GW802 - Large Vegetable Belt Slicer P/N: LG-BELT-SLCR; a) Employees were exposed to being cut by blades during the operating cycle when the safety guard of the scallion slicer machine was removed to clear product getting stuck on the conveyor. When the conveyor safety guard was removed, the conveyor belt moving the products towards the blades stopped. However the blades cutting operation continued, exposing the operator to inadvertently getting in close proximity to the danger zone. Safety switches/latches were not checked daily as per manufacturer's recommendations, to make sure they were properly working when the safety guards were removed.

29 CFR 1910.147(c)(7)(i): The employer did not provide adequate training to ensure that the purpose and function of the energy control program was understood by employees: Location: Fresh Cuts Produce area Slicer & Cutter machines; On or about 2/28/2018, 1) Employees performing maintenance activities such as changing the cutting blades were exposed to caught in between hazards. The employer failed to protect each employee by providing required training, periodic inspections and implementing procedures for the control of potential hazardous energy or other alternate measures which provide effective protection.
