29 CFR 1926.404(f)(6): The path to ground from circuits, equipment, or enclosures was not permanent and continuous: a) Jobsite, 210 Fowler Road, Warrendale, PA 15086: On or before February 28, 2018, an employee was using an extension cord to power a paint sprayer. The cord was missing the grounding terminal pin at its plug end.
