29 CFR 1910.28(b)(1)(i): The employer did not ensure that each employee on a walking-working surface with an unprotected side or edge that was 4 feet (1.2 m) or more above a lower level was protected from falling by one or more of the following: Guardrail systems, safety net systems, or personal fall arrest systems: Employees were exposed to fall hazards. Employees were exposed to three dock doors which were greater than 4 feet and employees were not protected from falling by guardrail systems, safety net systems, or personal fall arrest systems. On or about February 21, 2018, an employee was injured sustaining broken bones when the employee slipped off the dock to the parking lot below.

29 CFR 1910.178(q)(5): Parts of powered industrial trucks requiring replacement were not replaced with parts equivalent in safety with those used in the original design. Employees were exposed to struck by the load hazards as the Caterpillar propane forklift had a broken horn. A bicycle bell was being used on the forklift instead of the forklift horn being repaired. The forklift was not repaired with parts equivalent to those of original manufacture.

29 CFR 1910.1200(e)(1): The employer did not develop, implement, and/or maintain at the workplace a written hazard communication program which describes how the criteria specified in 29 CFR 1910.1200(f), (g), and (h) will be met: Employees were exposed to hazardous chemicals. Employees utilize chemicals such as but not limited to propane which is flammable, liquid calcium chloride which can cause skin and eye irritation, and other chemicals. There was no written program describing Labels and other forms of warning, how material safety data sheets would be maintained and employee training.
