29 CFR 1910.119(d)(3)(i)(B): The employer's piping and instrument diagrams were not accurate and did not represent equipment that was existing and was part of the process. At Plant B: The Piping and Instrumentation Diagram developed for the system is not accurate. The Air Unit 12 located at room 33 has been removed. The Piping and Instrumentation Diagrams were not updated after removal of AU12 in July 2017.

29 CFR 1910.119(j)(5): The employer did not correct deficiencies in equipment that were outside acceptable limits (defined by the process safety information on paragraph (d) of this section) before use Employees were exposed to an electrical hazard. Employees would not be able to pull the disconnect switch of the evaporator to do maintenance or shut down due to the amount of ice accumulated. Maintenance/Ammonia technicians employees are exposed to electrical hazard when they try to break the ice accumulated in the disconnect switch, the electrical box can break and loose the integrity.

29 CFR 1910.147(c)(5)(ii)(D): Lockout devices and tagout devices did not indicate the identity of the employee applying the device(s) At the Mechanical Room of Plant B: Employees were exposed to an electrical hazard. A lockout device used to isolate electrical energy on the evaporator panel did not contain identifying markers by authorized employee. Maintenance/ammonia operators working in the mechanical room were exposed to electrical hazards when locks and tags were not labeled and identified.
