The employer failed to ensure that each employee had the proper personal protective equipment in place before working.

The employer failed to provide any fall protection means to the employees on the scaffold system.

On or about February 27, 2018: a) At the jobsite located at Lot 115 of the Front door Communities subdivision, located in Cumming, GA, were exposed to fall hazards of approximately 16.7 feet, when the employer failed to remove the damaged ladders that were being used as part of ladder jack scaffold system, employees were utilizing to install siding material.
