29 CFR 1910.147(c)(1): The employer did not establish a program consisting of an energy control procedure, employee training and periodic inspections to ensure that before any employee performed any servicing or maintenance on a machine or equipment where the unexpected energizing, startup or release of stored energy could occur and cause injury, the machine or equipment shall be isolated from the energy source and rendered inoperative:  On or about March 1, 2018, employees were exposed to struck-by and amputation hazards when there was no Lockout/tagout program: a. There was no program describing the employers implementation of lockout/tagout at the site.  b. There were no machine specific lockout/tagout procedures for equipment with multiple energy sources such as, but not limited to, the HEM horizontal band saw.  Lockout/tagout is required when changing the saw blade. c. There was no effected employee training for employees that are not authorized to perform lockout/tagout d. There was no authorized employee training for employees that are authorized to perform lockout/tagout e. There was no annual review of the procedures and employees f. There was no program information for how lockout/tagout was to be done by outside vendors who serviced equipment such as, but not limited to, CNC equipment g.  There was no program information indicating that cord and plug equipment was to be unplugged when performing blade changes on equipment such as, but not limited to the DoAll band saw. h. There were no specific locks to be used for lockout/tagout  In accordance with 29 CFR 1903.19(d), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET), and in addition, documentation demonstrating that abatement is complete must be included with your certification.  This documentation may include, but is not limited to, evidence of the purchase or repair of the equipment, photographic or video evidence of abatement, or other written records.

29 CFR 1910.178(q)(7): Industrial trucks were not examined before being placed in service:  On or about June 26, 2017, employees moving material and equipment with the forklift were exposed to a caught by hazard when the backup alarm did not work.  On or about March 1, 2018, employees were exposed to struck-by hazards when the forklift that was used 1-2 times per week was not inspected prior to being used.  FIRST QUALITY MACHINE, INC. was previously cited for a violation of this occupational safety and health standard or its equivalent standard 29 CFR 1910.178(q)(7), which was contained in OSHA inspection number 1242574, citation number 1, item number 1 and was affirmed as a final order on 11/17/2018, with respect to a workplace located at810 Old Hull Road in Athens, Georgia.

Employees use chemicals such as, but not limited to, Neuco coolant.  No abatement certification or documentation is required for this item.

29 CFR 1910.1200(h)(1): Employees were not provided effective information and training on hazardous chemicals in their work area at the time of their initial assignment and whenever a new hazard that the employees had not been previously trained about was introduced into their work area:  On or about March 1, 2018, employees had not been trained on the hazards associated with chemicals that they used such as, but not limited to, Neuco coolant.  Employees had not been trained to know how to find and use the hazard communication program nor safety data sheets.  In accordance with 29 CFR 1903.19(c), abatement certification is required for this violation (using the CERTIFICATION OF CORRECTIVE ACTION WORKSHEET).
