In the loading area, where an employee was exposed to a struck-by hazard when operating a Toyota Forklift, Model 02-8FGJ35 with a 397.7-lb telescopic boom/slab lifter attached to the tines without the manufacturer's prior written approval.

29 CFR 1910.178(a)(5): The powered industrial truck was equipped with front-end attachments other than factory installed attachments, however the employer did not request that the truck be marked to identify the attachments and show the approximate weight of the truck and attachment combination at maximum elevation with load laterally centered: On or about March 1, 2018: a. In the loading area, where an employee was exposed to a struck-by hazard when operating a Toyota Forklift, Model 8FGCU25 with a 397.7-lb telescopic boom/slab lifter attached to the tines without the forklift being marked to identify the attachments and show the approximate weight of the forklift and attachments. b. In the loading area, where an employee was exposed to a struck-by hazard when operating a Toyota Forklift, Model 02-8FGJ35 with a 397.7-lb telescopic boom/slab lifter attached to the tines without the forklift being marked to identify the attachments and show the approximate weight of the forklift and attachments.
