An employee was injured when he fell while trying to access the roof bulk head from the ladder.

Within twenty-four (24) hours after the in-patient hospitalization of one or more employees or an employee's amputation or an employee's loss of an eye, as a result of a work-related incident, you must report the in-patient hospitalization, amputation, or loss of an eye to OSHA. Site: 340 East 64th Street New York, NY On or about 2/25/18 a) Employee was injured when he fell from a ladder. He was hospitalized for approximately 4 days. The employer did not notify OSHA as required by the regulation.
