Location: 391 Hooper Street, Brooklyn, NY; 6th Floor.  On or about 3/1/2018  a) Employees were not able to recognize safety hazards such as, but not limited to, using a grinder without guard, cords with missing ground pins, and scaffold platform not gully planked and missing guards rails.

29 CFR 1926.303(b)(1): Grinding machines shall be equipped with safety guards in conformance with the requirements of American National Standards Institute, B7.1-1970, Safety Code for the Use, Care and Protection of Abrasive Wheels, and paragraph (d) of this section.  Location: 391 Hooper Street, Brooklyn, NY; 6th floor.  On or about 3/1/2018  a) An employee used a Chicago Electric angle grinder to cut rebar and c-joists.  The grinder was not equipped with safety guard.

29 CFR 1926.451(b)(1): Each platform on all working levels of scaffolds was not fully planked or decked between the front uprights and the guardrail supports as specified in paragraphs 1926.451(b)(1)(i)-(ii)  Location: 391 Hooper Street, Brooklyn, NY; 6th Floor  On or about 3/1/2018  a) The platform assembled on the 6th frame of a supported tubular scaffold was not fully planked.  One employee used this platform to provided hand signals to the crane operator during a delivery.
