29 CFR 1926.453(b)(2)(v): A lanyard was not attached to the boom or basket when working from an aerial lift. On the west side of the building, an employee, not attached to the basket worked from Sky Jack work platform installing sheet metal gutters was exposed to fall hazards of approximately 12 feet to the ground below.

29 CFR 1926.501(b)(1): Each employee on a walking/working surface with an unprotected side or edge which was 6 feet (1.8 m) or more above a lower level was not protected from falling by the use of guardrail systems, safety net systems, or personal fall arrest systems. On the south side of the construction site, an employee without fall protection worked from an overhead balcony was exposed to a fall hazard of approximately 10 feet.
